Co-Payment will be required at the time of each visit. Payment can be made by Cash, Check or credit cards (we honor VISA, MC and Discover).

Please confirm with your Insurance Company to verify for your individual plan type. For any reason, if we were not able to bill it to your insurance company due to non-coverage, you will be charge for Doctor’s fee and you will be responsible for billing your insurance company.
